The FSUCard Center would like to remind all departments,
faculty and staff of the required documentation for new faculty
and staff to obtain I D Cards. Only the following
will be accepted.

Copy
of the employee job information page from the OMNI
system or |
| 2 |

Copy
of the employee offer letter from the department or |
| 3 |
|
Copy
of the employee contract or |
| 4 |
|
Letter
on departmental letterhead, stating job code, job
title and employment starting date. The letter
must be signed by an authorized representative
of the department. |
Anyone applying for and FSU ID
Card must have
a government issued photo ID. Any one of the following are acceptable
